HealthSnap has secured $25 million in financing led by Eastward Capital Partners to expand its artificial intelligence-powered virtual care platform across United States health systems. The funding will support product development, commercial growth, and the company’s Advanced Primary Care offering while also refinancing existing debt.
The Miami-based company plans to use the capital to advance its AI capabilities and broaden enterprise adoption of its healthcare platform. HealthSnap recently launched an AI-enhanced Advanced Primary Care Management solution that combines agentic AI with clinical workflows to help healthcare teams manage larger patient populations more efficiently.
The platform integrates Remote Patient Monitoring, Chronic Care Management, Principal Care Management, Advanced Primary Care Management, enterprise analytics, reimbursement optimization, and care coordination into a single electronic medical record-integrated system. Chief Executive Officer and co-founder Samson Magid said the funding validates HealthSnap’s long-term vision of making continuous, intelligent healthcare the standard across modern health systems. He emphasized that artificial intelligence is designed to strengthen clinical teams rather than replace healthcare professionals.
HealthSnap’s platform currently processes two patient measurements every second and has reduced clinical alert frequency by 97 percent for patients after their first year of enrollment. The company supports more than 80,000 active patient programs across 200 health systems and physician organizations, with expectations to exceed 100,000 active care programs by the end of 2026.
Health system partners include Prisma Health, AdventHealth, Ascension Health, Sentara Health, Tampa General Hospital, UnityPoint Health, Baptist Health South Florida, and Mount Sinai Medical Center. Through 2025, the company’s AI platform assessed over three million clinical progress notes, extending quality assurance beyond manual audits. AI-simulated patient interactions are also being used to train newly hired nurses, contributing to a Net Promoter Score of 72 while improving both employee and patient engagement.
The platform generates clinical summaries to streamline disparate patient information for faster clinical decisions, while AI agents assist with patient enrollment, device support, and compliance outreach. Over the past three years, HealthSnap has achieved a fivefold revenue increase and steady year-over-year growth. The company received a 2025 MedTech Breakthrough honorable mention for clinical workflow and operational proficiency and was named to the Inc. 5000 list of fastest-growing American companies for 2025.
Investment proceeds will further expand HealthSnap’s teams of artificial intelligence specialists, engineers, and product developers to fuel growth into existing and new healthcare enterprises globally.
